After a period of time, character avatar glitches

After playing for a time with caves and cliffs enabled on PS4, the character will begin to mine and the blocks do not become harvest able. As in they just disappear when mined. Afterwards, the avatar begins glitching through blocks, unable to jump upwards, and going underwater causes no air lost.
