My game freezes for unknown reasons.

My game freezes every time i play the game for like 40 minutes the game freezes and it does that every second or minute depending on how long i play and it only affects me in my world and how i know is when i freeze after jumping my friends describe seeing me float and the game also freezes on the main menu screen or loading screen or in servers and i don't have a clue what could be causing the problem and the only way i could unfreeze the game for a slight amount of time is pressing my home page button on my xbox contoller.
