Instant death when leaving end

i jumped into the end portal to leave the end, then when I respawned I was dead. I closed the game and reopened it and all my stuff was gone and I only had 7 levels. I didn’t save and quit because when I hit respawn, the options disappeared and nothing happened.

the bed was on solid black stone blocks. It was near carpet, but not that close. Any idea why this happened???
