I die in portals and lose everything

So this has happened twice. I am full health is survival. When I enter a portal i will suddenly die and lose all of my stuff. First time was in the end young to the overworld i jump in the portal died and was stuck in loading scene and couldnt access the options scene to log out or anything. I had to get a second remote to save me. The second time was recent and i went from the otherworld to the neather and i dead. I had full heath and full diamond armor. I died and lost everything while i was in the loading scene. Since this has happened twice now in the same world i am reporting it.
