Nether portals are broken

Recently, whenever I enter a nether portal, there is a random chance that I will be teleported a massive distance from where I placed the portal. In some cases, a portal wouldn't even spawn behind me, thereby leaving me trapped in the nether. The chances of this event occuring seem to be at their highest when I first load up my world, to the extent that I am often afraid to use portals.
