Placing Axolotl Gave Into Fire Achievement

My sister was playing Minecraft and she placed an axolotl in a 3x3 pool of water 2 to 3 blocks deep. She immediately received the Into Fire achievement, despite having never killed a blaze in survival. After closing the app, the achievement is still there. This happened on an iPad 6 hours after updating to 1.17.
