Strange item

Hello Mojang, I was exploring a mansion in the forest, and in one of the rooms, specifically in the "Tree felling room", an ax with Efficiency I did not appear, as it usually is, instead a leather helmet with Efficiency I appeared. I don't know if it is an error or an extremely small probability of this happening.
