Unable to Recover Items after Death

After I die I go to pick up my dropped items and I'm unable to pick them back up. I loose either All or Half my inventory and all I can do is watch my items De-Spawn. This has been happening for a Week and it's been very hard to play when an issue like this keeps discouraging me to play in fear of loosing something valuable. The items just float then dissappear after a while.
