Save the game in the nether and when you respawn you're dead and all your stuff has disappeared.

It's happened about 3 times that I save the world while in the nether and when I load up the game again it says that I've died but when I had logged out I was at full health. I respawn and when I go back to the place it shows I died all of my stuff has disappeared. There's no way to get my things back. I'm not sure how to get it to happen though. It randomly does this. And I haven't been able to intentionally recreate it.
