Respawn anchor doesn't work

It won't work on the first death but will every time until you have to charge it again and it's a very consistent bug I tried it multiple times. I logged off of the world and went back on and can no longer reproduce it and it just doesn't work. I placed it in my nether base on netherrack that is under a lava ocean surrounded by glass blocks. when it was working, every so often it would spawn me in the glass.
