Killed by Mobs or falls, and I can't pick up my blocks when I find them

So I downloaded the Minecraft update a couple of weeks ago.

 After that, the game worked pretty well all in all, but for the random occasion's where I am killed by mobs or from falls, the items I drop cannot be picked up again no matter how close I am to them.

 In the attachment, is the screenshot from my PS4 for that I took standing very close to the items I dropped.
