Blocks break but no sound or drops, then they reload and you suffocate

Breaking Netherrack but no breaking sound or drops, then the blocks reload and you start suffocating. Died multiple times to this, and lost a set of perfect Netherite armour, sword and pick. Also have died with other gear in other worlds
