Impossible death, inventory despawned

I had just cleared out an area of stone and filled it with grass. I was standing on a pressure plate, trying to go in my base and was disconnected from the game, then was despawned at the spawn location. I had no items on me, then I member of the realm let me know it said I was suffocated in a wall. I noticed all the grass I had laid was stone again & none of my stuff was in the area, anywhere. So I died an impossible death & lost EVERYTHING. I'm running on 1.16.2. I've never had Minecraft make me so mad
