Spawning in sky at game start

Every time I create a new world, or if someone joins my world for the first time, they will spawn in the sky and fall to their death.

Steps to reproduce:

1) Create a new world

2) Play the world (or join as a new player)

3) Fall to death

4) Respawn on the ground
