I died and all of my un-equipped items disappeared.

I was fighting iron golems in an underground room when I was cornered and killed. I re-spawned in the same room and went to retrieve my items when I discovered that only my armor and equipped shield were still there. I dug around in the wall, looked on the surface, nothing. It seemingly vanished. I had some rare items and have no idea where they could have gone.
