My game is laggy/glitchy

My game has been laggy. I have been okay with it for some time but now it getting on my nerves. The lag effect the whole game. I love the game but please help. The descriptions are:

• Freezes every 1 second.
• happens often.
• causes me to get kick out.
• When kicked I come back in a place that I was in 1 to 6 minutes ago.
• In settings It lags and often changes something I want or don't.
