Tools and ore keep disappearing from chest

Ive noticed my tools and ores keep despawning from my chest when reloading my world. 

I had diamond and emeralds, iron and redstone in my chest along with tools and armor.

Not sure how to properly replicate this. 

You can see in my screeen shot the empty spaces where something was.
