Client-side hit detection.
When a player is lagging internet-wise and hit a player when the player is out of range, the server still registers the hit. Also, other things like inventory seem to be client-side, leading to cheats ruining servers. I have been trying to improve in Minecraft PvP, but I get killed by laggy players.
