Relogging in lava/fire teleports the player to a random place

Summary: If you leave the game while standing inside of lava or fire, then rejoin the world, you will be teleported to some random spot.

Impact: If you get teleported outside of the bounds of the map, you have softlocked yourself and the map is now unplayable. Therefore, any time these blocks are used, the player can softlock at any time. We can't reasonably protect against the player literally randomly teleporting.

Repro Steps:

1. Place some lava.
2. Stand in it.
3. Close and reopen the world

Expected Results: You are exactly where you were before.

Observed Results: You teleported to some random spot.
