Resource and Behaviour packs being randomly uninstalled from players worlds

Summary:
This issue has been reported on multiple maps, across Xbox One, PS4 and iPad editions, though there was a significant spike in reports in April 2020 following the release of 1.14.60 and Millionaire Mansions 2. There have been no confirmed reports on Windows 10 edition at the time of writing.
 
Players report starting the world from the world template as normal, playing the world as intended for a bit but after closing the app and reopening their world they find that the resource or behaviour pack or, very rarely, both have been uninstalled without warning or any player involvement. This has lead to confusion and some distress to younger players.
 
Our current recommendations to fix this issue has been to ask players to go into the resource and behaviour tabs manually and make sure the packs are active. Occasionally it's been reported that the packs have disappeared altogether. In those instances, we have recommended clearing the player's cache and redownloading the pack. This often seems to work as we've had no reports of packs failing after re-instalment.
 
As we've been unable to replicate the issue in any of our tests, I've compiled the reports into one document with all the details I've been able to gather, as well as some pictures from players as references to what's going on.
 
Impact:
We received several emails from people telling us about pack missing
Since we couldn't reproduce it, we've compiled a document which we are continuously updating with reports.

These pack loss issues will result in lower ratings, negative social media, more customer complaints, and potential refunds.
