GAME CRASHING AGAIN!

Hello, everytime I play minecraft I join a server. After 5 minutes of playing my game will crash and say most of the time "The game crashed whilst tessellating block model". What does that even mean? And then when I press the "View crash reports" button it wont even open or do anything. This happens on every server I join. Even sometimes on single player. Please help! Ive had this issue for 3 days already!
