Diagonal screen tearing in upper left corner

From time to time i see weird screen tearing, it diagonal and only appears in left upper corner of the screen. It makes taht corner tear into two triangles. it shows regardless of vsync off or on. Unfortunally I can't capture it in action but it can be reproduced but rapidly clicking Esc to get to pause meniu and out.
