Rendering problems, only mobs, animals and GUI (not items) render

Can't explain properly the issue but there are screenshots!
This bug affects 1.8 and up.

Using java 8 version 66
AMD Mobilty 4500/5100 HD
4 GB RAM
Ingame Video Options don't matter.
Windows 7 Home Premium
