blocks glitching

it happens on any server or single player I could be placing blocks down or eating or killings something and then all of a sudden blocks start to disappear and reappear and coming off and then back on the server or single player world just makes matter worst and its coming to a point where I don't want to play minecraft because of the amount on annoyingness
