Hopper/furnace losing items

When I have a hopper connected to a furnace which is connected to another hopper leading to a chest. Sometimes the fuel and items being cooked vanish and the furnace still looks like it is cooking with nothing in it. It can be restarted however items are gone and not in the hopper or chest below the furnace.
