When combining items on an anvill the items disappear

i put in a pick with fortune 3 and efficiency 4 and a pick with unbreaking 3 and efficiency 4 to make a fortune 3 efficiency 5 unbreaking 3 pick but when i go to grab the pick out of the 3rd slot all picks disappear
