Nether Portals broken in Survival Mode

When in survival mode, nether portals will teleport you to inside nether terrain, or in mid air, and will not create a connected portal in the nether. The same problem happens when you build a portal in the nether and try to get back to the overworld. Problem does not seem to exist in Creative Mode.
