Sometimes players are not invisible even under the invisibility effect

Whilst building my work in progress PvP map, I had this glitch where even when I gave the player the invisibility effect, the player would not become invisible. I cannot reproduce this, as after reloading the map the invisibility effect did work, but I do have some screenshots of the issue.
