Body becomes permanently twisted after a while

My apologies, I have not figured out exact steps to reproduce yet. But I was able to induce it happening multiple times (with client restarts in between) by playing around with placing blocks from the offhand, from the main hand and using the bow with and without something in the offhand. See attached screenshot for how my torso is now twisted.
