Potions aren't converted from <1.8.x to 1.9

All potions using the previously called "damage values" aren't converted to the new potions using NBT tags, rendering them all useless.

As the provided screenshot shows, the previous potions are rendered as an invalid resource icon, and don't have any understanding from the game.
