Weird graphics

What I expected to happen was...:
I don't know, I play for 3 minutes and this happens

What actually happened was...:
Graphics go crazy and totally weird

Steps to Reproduce:
I don't know how to reproduce it, just happens...

I formatted my pc 3 times, changed my processor, my ram, my mother board, and my hd, the only thing I don't changed in my pc is the Video Card (GT 520 1024Mbs)...

I leaving my dxdiag and msinfo32 for my pc specs...
And sorry for the bad english, i'm brazilian and I have 16 years old...
