player falling through all blocks, xray vision

player logged on and was falling through all blocks. console showed the logon but then a logoff even though they remained connected. tab no longer showed the player. x-ray vision like images and the ability to pass-thru everything. When player logged off there was a logoff message immediately followed by a logon & player was back to normal.
