server message that a player has left but they are still in game

Chat will show a 'player left" message and tab will no longer show the player but that player is still able to function in-game & interact with blocks while invisible to other players. Logging out & back resolved but it keeps happening. No messages other than the player left appear in the console. When the player logs out to reset, no messages appear.
