Hopper making duplicate items

If you have 2 hoppers above each other throwing an item into the top can sometimes cause 2 items appearing in the GUI of the second hopper. It's like a 1/4 chance. And the items are not ghost items as they're placeable on the floor. It does it on creative and survival.
