Minecraft crashes upon death and keeps crashing if you try to reload the world (also affects servers)

I died in game and so expected to respawn at the usual place (my bed) but instead minecraft crashed. I try to reload the world and it just keeps crashing. This also happened to me on a server, when I try to re-log into the server, Minecraft crashes again.

Upon reloading the world, I seem to spawn, but everything around me is dark, after about half a second, it crashes.

Both deaths were from fall damage.
