Dungeon chests and spawners don't spawn correctly

There should be chests and a spawner in dungeons.

When I found a dungeon, the items were on the ground and there were no chests, instead there were cobblestone where the chests should be. And there was no spawner but the spawner particles were on spot where the spawner should be.
