Far Left Slot item disappearing

My item bar would be full with all slots empty, when I would go into my inventory the item on the far left would disappear or rearrange itself into my inventory instead of staying in the far left slot. This is only affecting that one second of the slots. When I would put any item back into it would be fine until i went into the inventory
