Zombie head glitch

I used a downloaded map I made I made a zombie spawner that spawned a baby zombie with a player head on it; it did not go on the zombie's head it was floating. Also when I downloaded the map all the mobs with items such ass spawn eggs or splash potions had normal eggs and empty potions.
