Weapons disappear and can't break blocks

I uploaded some pictures to show what I mean, the sword seems to be out of frame and breaking blocks is impossible, seen this happen with a bow too, happens randomly and disappears randomly too from what i've seen, only running not enough items and minecraft forge, both claiming to have nothing to do with this
