Disappearing Items

I was riding in a boat and then lost it, running through water and two shovels disappear out of my inventory, and using my axe to take out oak wood planks to replace them with birch planks and 3 out of the 5 oak wood planks disappeared and not even in my inventory? Definitely confusing!
