Most of the time when i reload a world my recently placed blocks will be gone , and recently destroyed blocks will be back.

Most of the time when I reload a world any blocks I recently placed will be gone , and any blocks I recently broke will be back.I can not tell you how to recreate it , 1 day it just started happening , but i know it started in 1.3.2.
