Offhanded cobblestone got deleted when stone axe broke fighting zombie.

I had blocks in my offhand and upon killing a zombie my stone axe broke and when that happened the blocks in my offhand got deleted. I use lunar client if that is relevant it MAY be an issue with them but I have never had something like this happen playing unmodded minecraft.
