Unable to get bad omen from killing pillager captain

Upon killing the pillager captain at an outpost or from a pillager patrol in my hardcore world, the captain does not drop any ominous banner nor give me the bad omen effect. Hence until now I have still not obtained the voluntary exile achievement and cannot start raids at all. The game crashed the first three times I killed the captain (log attached below). At first I figured it was because of the fire damage from my sword so I switched to my axe, but still did not get the effect.
