When an enchanted tool is used up, it sometimes creates a ghost tool that keeps reappearing

I think this is only with efficiency tools, but when you have used up the tools' last use and they break, they will almost always keep reappearing as tool with one use, then you use them again and they break, then they reappear, then they keep breaking, etc.
It's a very old bug.
