Ghost items are created by loot pots

Items can still be placed inside of loot pots in the Trial Chambers. It disappears from your inventory and plays the shake animation. But since there is already loot in the pot, it can't go in, creating a ghost item. By pressing Q, it recognized I still had the item and spat it out. Then, I broke the pot to prove there was loot in it.
