You don't take "outside_border" damage if you are taking suffocation damage

Steps to reproduce:
1) Go into spectator mode at the world border and go into survival mode after going as far as possible out
2) Compare this damage taken inside blocks, outside the border

Expected:
The suffocation and outside_border damage stack

Observed:
You only take the suffocation damage
