Lost my inventory entirely upon death

We are playing Vault Hunters update 9.0.3, I was attempting to use a boat and a zombie killed me. In this modpack, your inventory usually drops a gravestone if not in a vault, however this time it did not. I had full netherite gear including an axe and pickaxe, an inventory full of cobblestone and some other odds and ends.
