crashing when going through portals

Every now and then when I go through a portal I crash.

Crash message: EXCEPTION_ACCESS_VIOLATION (0xc0000005) 

I have tried redownloading java, still wasn't fixed.

I tried windows memory diagnostic, there wasn't any problems (still wasn't fixed).

(everytime Minecraft dies doe to this problem, no crash reports are given)
