Stuff disappears after death and respawn no where to be found again.

All of inventory is lost randomly after death and respawn with it no where to be found. I have been seeing this bug a few times aspesually after a new update.

What I expected to happen was...:
That I'm able to retrieve my stuff after death and respawn

What actually happened was...:
When I'm killed there seems to be a very high chance of me looseing all my stuff that I had in inventory.

Steps to Reproduce:
1. Run minecraft afk or as present play
2. ... be killed by mob 
3. ... do not respawn right away after death
