Whenever I mine a block somewhere (not sure exactly what causes it) - me and the shield swap places

So I wanted to mine sand in my hardcore world where suddenly I noticed that the part of that beach isn't loaded properly and me and the shield in my offhand has swapped places: I saw myself in front of my screen and me running in my offhand (view videos for reference)
